This is the individual player / rostering registration form.  All players must complete it.  If you are playing on 2 different teams then you must complete this form for each team that you are playing on.  

Only players who are listed on the official game sheets will be allowed to participate in the USBOXLA Nationals games.

This individual player / rostering registration form automatically populates the game sheets for the event.
